Diet is rapidly moving from alternative treatment to essential medicine. A growing number of healthcare providers are using food to help manage chronic conditions like diabetes, heart disease, hypertension, and kidney disease. And Food as Medicine is gaining traction with health insurance plans, including Medicare and Medicaid.
Sifter is the Food as Medicine Platform.

But many of these Food as Medicine benefit programs have serious limitations. Although medically tailored meals (MTMs) have proven effective in improving health outcomes, they're expensive to administer, which is why insurance typically covers this benefit for only 12 weeks. Patients needing continued nutrition support can be transitioned to produce prescription (PRx) programs, but these are often hampered by inaccurate and incomplete grocery product identification, frustrating recipients with unnecessarily limited produce eligibility.

The answer is Sifter's Food as Medicine platform. Our algorithms identify grocery food eligibility for hundreds of chronic health conditions, including diabetes and heart disease, as well as food allergies, cultural preferences, and even ingredient likes and dislikes. We use these science-based, dietitian-vetted algorithms to build a benefit program's approved product list (APL) — the foods that recipients are authorized to buy. These custom APLs, defined at the UPC level, are highly accurate, curated by health condition, and include grocery products across all categories (frozen, canned, dried, fresh), offering cost savings and convenience.

Using Sifter's platform, healthcare providers, payers, and employers can transition patients and members to medically tailored grocery (MTG) programs that are flexible, scalable, and cost-effective, including:

• Highly customized digital spending cards that can be used to buy only pre-approved foods across a network of national and local food retailers;

• Online MTG "storefronts" linked into your existing site and showing groceries eligible for specific health conditions;

• A barcode scanning tool that can be easily integrated into any mobile app. Recipients use their smartphone to scan grocery products and learn, on the spot, whether the food will be covered by their benefit.

Healthy eating made easy. Sifter helps build grocery benefit programs that increase engagement, improve health outcomes, and lower healthcare costs.
